Abstract

South Africa’s National Health Insurance (NHI) policy framework represents a landmark reform aimed at achieving universal health coverage (UHC) and addressing persistent inequities in access to healthcare. Despite its transformative intent, the policy has encountered profound challenges that extend beyond fiscal constraints or procedural implementation, encompassing institutional, structural, and governance dimensions that undermine equitable and sustainable healthcare delivery. This study focuses specifically on the South African NHI policy framework as a case, critically examining the structural barriers embedded within the policy that constrain its effective implementation. The study thus contributes to a more nuanced understanding of the systemic and governance determinants shaping health reform in a post-apartheid context. A qualitative research design was employed, combining semi-structured interviews with document analysis of official policy documents, annual reports, and oversight evaluations. Ten purposively selected stakeholders from national and provincial government departments, Parliament, Non-Governmental Organisations (NGOs), the private sector, and academia were engaged. Data were analysed thematically using Institutional Theory, Policy Diffusion Theory, Health Systems Theory, and Policy Implementation frameworks. This analytical approach enabled an examination of how institutional norms, bureaucratic structures, and governance arrangements embedded in the NHI policy framework influence reform outcomes. Triangulation of interview data with policy documents enhanced validity and contextual depth. The findings identified five interrelated structural barriers within the NHI policy framework: (i) weak intergovernmental coordination and institutional fragmentation, (ii) legislative and policy ambiguities undermining implementation clarity, (iii) insufficient stakeholder accountability mechanisms, (iv) fiscal and human resource capacity constraints, and (v) entrenched structural inequities resulting from South Africa’s dual health system. These barriers illustrate that the challenges facing the NHI are structural and systemic, reflecting policy design limitations, overlapping mandates, and bureaucratic inertia, rather than solely technical or operational deficiencies. Based on these findings, the study recommends establishing an Intergovernmental...
