Abstract
Obesity poses a significant threat to public health, both nationally and globally, due to its high
prevalence, incidence, and economic burden. This condition is caused by an excess of fat in
the body, which increases the risk of several diseases. In South Africa, obesity is becoming a
concerning public health issue. Its prevalence is on the rise and is a significant health concern
in sub-Saharan Africa. Radiographic imaging is a vital tool in healthcare for investigating,
diagnosing, and treating medical conditions. However, performing radiographic imaging
requires standardised procedures to ensure accurate results based on average-sized patients.
Difficulties may arise when patients with obesity are being managed and while performing their
radiographic procedures. Patients with obesity require modifications to routine radiographic
practices, including movement, assisted positioning, and equipment capabilities to
accommodate increased weight or coverage. Additionally, patients with obesity may
experience compromised radiological diagnoses due to poor visualisation of structures and
may also experience increased radiation dose due to repeat examinations. The diagnostic
radiographer must navigate these limitations while striving to achieve optimum diagnostic
images and maintain a high standard of patient care and dignity.
This research aimed to gain insight into the perceptions and experiences of diagnostic
radiographers when imaging obese patients. A qualitative, exploratory and descriptive
approach was utilised, employing a non-probability purposive sampling technique to select a
sample of diagnostic radiographers working in public hospitals in Johannesburg, Gauteng
province, to participate. Twenty-five unstructured, individual interviews were conducted until
data saturation was achieved. The interviews were then transcribed and thematically
analysed. Codes and themes were developed using both inductive and deductive reasoning.
An independent coder also analysed the data separately to ensure credibility.
To achieve trustworthiness, the study incorporated several strategies, including credibility,
transferability, dependability, and confirmability. The themes developed in response to the
research question highlighted that significant diagnostic limitations were encountered by
participants, particularly related to the equipment and the positioning of obese patients during
imaging. Furthermore, participants noted that determining appropriate exposure factors for
obese patients posed a technical challenge that often compromised image quality, frequently
necessitating repeated imaging. They also expressed concerns about the emotional,
psychological, and physical demands associated with imaging obese patients.
